β¦ Synopsis
At present, vasectomy is the most common cause of obsmctive azoospermia. There are three major aspects to the problem of vasectomy reversal. The first concerns techniques for obtaining a reliable reanastomosis of the vas deferens. With modem microsurgery, accurate reanastornosis &odd be achievable in almost every case. The second aspect of the problem relates to the detrimental secondary effects of vasectomy, i.e., the pressure-induced epididymal damage occurring secondarily as a result of the vasectomy.
The third aspect of the problem is how to bypass this secondary epididymal obstruction microsurgidy.
It is advisable to begin practice in the animal before doing such surgery on humans.'-'* For the best mucosal approximation in the human situation when lumens are of different diameter (because of chronic obstruction and increased pressure), I recommend a nonsplinted two-layer approach. With a one-layer anastomosis, there is poorer mucosal approximation when lumen diameters differ. A splint of any kind should never be used and is only an excuse for not being certain one has obtained a good anastomosis. It results in sperm leakage, inflammation, and more scarring.

Infections of the male genitourinary tract may contribute to infertility by adversely affecting sperm function, causing anatomical obstruction or initiating a leukocyte response. The majority of infertile males are asymptomatic, and the significance and the criteria for the diagnosis of a genital tr
