In the letter by Wallace et al. ''A Stitch in Time Which Is
Perfectly Aligned Saves Nine'' (Microsurgery 2008;28: 392-393), I was pleased that they referenced my article 1 on the use of microdots for precision suture placement at the time of vasovasostomy. The authors state, ''others 1 reported using eight equally spaced dots sited radially on the cut ends of the abdominal and testicular remnants of the vas deferens to define placement of microsutures. However, this does not guard against twisting.'' In fact, if one actually reads the article and sees the illustrations, in Figure 2 of the article, one sees that the dots are placed at the 3 o'clock and 6 o'clock positions of the vas deferens with lines extending from them to allow perfect alignment of the ends of the vas (Fig. 1). In addition to allowing anastomosis of lumina of unequal diameters without ''dog ears,'' the lines extending from the microdots at the 3 and 6 o'clock position assure perfect alignment. This same system could be easily adapted to anastomosis of vessels involving lumina of unequal diameters to prevent twisting and ''dog ears.''
