Randomized controlled clinical trials (RCTs) usually aim to demonstrate the ecacy of drugs under a speci®ed set of conditions. Trials are designed to meet the needs of sponsors, regulators, and to some degree investigators. They are often poorly designed to meet the needs of prescribers and consumers of modern medications. In part, this is because a trial designed to demonstrate ecacy, in an ecient manner, may not provide satisfactory information on the overall balance of bene®t and harm. In particular, there is likely to be inadequate information on the adverse outcomes of treatment with the new medication. 1 With drugs
