A 4-year-old boy with orbital rhabdomyosarcoma developed central nervous system (CNS) metastases as a meningeal disease without clinically apparent local recurrence. Abnormal cells in the cerebrospinal fluid (CSF) antedated the symptoms and signs of increased intracranial pressure. This suggests that the CSF of patients with rhabdomyosarcoma should be monitored and that prophylactic therapy of the CNS might be indicated, especially in those patients with head and neck or metastatic disease.
