The Institute of Medicine's (IOM) report, To Err is Human, 1 published in November of 1999, exposed the frailties and the realities of the healthcare delivery system. Though the hairsplitting about the data presented in the first IOM report continues today, and healthcare practitioners and researchers still argue about the precise number of patients that suffer or die from medical errors 2 , many would suggest that the actual number is less important than the fact that anyone dies needlessly from preventable medical error.
