Recent improvements in gray-scale sonography have increased its usefulness in evaluating the pediatric urinary tract.' Sonography will readily demonstrate alterations of renal parenchymal echogenicity in a variety of disease states,2-4 but will rarely define the exact etiology of the parenchymal abnormality. We report a newborn with intensely echogenic renal pyramids suggestive of medullary nephrocalcinosis.
