โœฆ Synopsis

Metastatic disease in the neck from an unsuspected primary still represents an interesting and challenging dilemma for a physician. For the patient the implications are profound and may be devastating since an incorrect solution may result in delayed or inappropriate treatment and affect a potential cure.

Because masses in the neck are so common and so few of them represent malignant disease, two extremes of treatment are often prescribed: watchful waiting or the deplorable practice of "shooting from the hip." An example of the latter is surgical removal of the mass as the initial diagnostic step even before undertaking a comprehensive history and physical examination, including a careful examination of the head and neck.
