โœฆ Synopsis

Six hundred seventy-one first, second and third orthotopic liver allografts in 520 patients were reviewed to determine the effect of donor-recipient mismatches or incompatibilities for the ABO blood groups on graft survival. A significant advantage for ABO donor-recipient identity was found, especially in adults for first grafts. However, a surprisingly large number of ABO incompatible grafts were successful. We recommend that nonidentical or incompatible grafts be limited to patients such as small children for whom the supply of available donors is severely limited for patients in urgent need of transplantation or retransplantation.

Clinical experiences with ABO blood group-incompatible hepatic grafts have shown that, unlike kidney and heart grafts, the liver is more resistant to antibodymediated hyperacute rejection although cases of hyperacute rejections have been reported. ABO blood group compatibility has been regarded as an essential prerequisite for successful adult living donor liver transplantation (LDLT). Novel strategies for overcoming the ABO blood group barrier, however, have markedly improved the results of ABO-incompatible (ABOi) LDLT. We describe our strat
