Some of the case reports describing presumed lithiumlneuroleptic interactions are considered. Lithium toxicity is proposed as a possible mechanism. The paucity of data provided on the 12-hour serum lithium levels should be noted. Most case reports do not specify the time interval between the last lithium dose and the time of venesection. Also, inadequate consideration seems to have been given to individual pharmacokinetic variability and demonstrates an apparent lack of awareness of the physiological time-course of sub-acute lithium toxicity.
