In a recent issue of this journal, Bagliolo et al. report a case of spontaneous non-Hodgkin lymphoma (NHL) and idiopathic myelofibrosis (IMF) [I]. We and others [ 2 , 3 ] have described the occurrence of multiple myeloma, a lymphoproliferative disease with a well-known immunological basis, in the course of IMF.
