The Los Angeles pediatric medical community attempted to ascertain if the needs of children were being met by the emergency medical services (EMS) system. Information was gathered from three sources: data were reviewed on pediatric trauma victims included in a trauma survey in Los Angeles County; records were retrospectively collected on pediatric utilization of mobile intensive care units (MICU); and a prospective study of paramedic runs involving children was made. The trauma survey revealed that head trauma was the most common type of trauma in children. It occurred in 40% of pediatric victims versus 28% of adults. The death rate in the field or emergency department was 12% for children versus 7% for adults. Sixty percent of the trauma, but 70% of deaths, occurred in areas in which there were no pediatric centers. The MICU and paramedic run records indicate that 10% of calls involved children. Fiftyfive percent were for medical problems. Death occurred in 2.7% of children. Of pediatric calls, 25% involved children under the age of 3 years, and 10% were less than 1 year old. Based on these patterns of utilization, the authors suggest that greater attention may need to be given to pediatrics in the training of medical personnel. They assert that the needs of children in the prehospital setting are not being met, and that the pediatric community must become more involved in EMS.
