peated once at 4 h), dexamethasone (6 mg/kg IV repeated at 4 h), or a nonsteroid control in the treatment of septic shock. Patients were assigned randomly if they had strictly defined hypotension unresponsive to crystalloid fluid challenge, decreased organ perfusion, and bacteremia or an identified infection source. Excluded were patients with hypo ~ tension secondary to hemorrhage, acute myocardial infarction, cardiopulmonary arrest, or aspiration. Shock reversal at 24 h was significantly greater in the steroid-treated groups as compared with controls; overall, however, numbers of reversed shock states were not significantly different. Mortality was significantly improved in the steroidtreated groups at 133 h, and until 150 h, after treatment. Overall mortality was not significantly affected. Significant differences were not observed between the different steroid preparations used. The only recognized complication in the steroid-treated groups was that superinfection occurred more frequently. The authors conclude that corticosteroids may improve short-term survival and be beneficial in certain subgroups of patients. Future studies should be directed toward identifying such subgroups.
