Sixteen cases of intracranial hemorrhage associated with amphetamine abuse are reviewed: Most patients experienced sudden onset of severe headache shortly after amphetamine use. An altered state of consciousness was rapidly followed by hemiparesis in almost all cases. Four of the five subsequent deaths occurred within hours following admission, and all survivors except one had permanent neurologic sequelae, such as hemiparesis, dysphagia, or seizures. The authors review several possible etiologies, such as necr0tizing angiitis from chronic amphetamine abuse, impurities of intravenous injection, incidental trauma, or cerebral embolization possibly secondary to abnormal ptatelet aggregation.
