Features unique to the four patients who underwent urgent OLT were grade I or grade II encephalopathy, gastrointestinal hemorrhage, peak prothrombin time (PT) of more than 50 (81 + 11 seconds) and platelets less than 90 K (70 +-8 K). In addition, metabolic acidosis was present in three and transient hypoglycemia in two despite supplemental glucose. Resected livers showed less than 5% viable hepatocytes in three and less than 30% in one patient. The single patient not requiring OLT had a peak PT of 22.5 seconds and a nadir ptatelet count of 112 K. Potentially useful selection criteria for OLT include a peak PT of more than 50 seconds in association with gastrointestinal hemorrhage, thrombocytopenia, hypoglycemia, and acidosis.
