She might have recovered without nitroglycerin but the reappearance and disappearance of peripheral pulses prove that vasodilation can affect the greater arteries in ergohsm. Similarly, a few cases have been reported where vasodilation with sodium nitroprusside infusion in ergotism was briefly stopped with reappearance of symptoms in the treatment-free interval (1).
The course of ergotism is not always benign, so when confronted with a patient with an un redictable risk of gangrene we would recommend a triaf treatment with a vasodilator. Because of the simple handling and the few side effects we consider nitroglycenn infusion, as suggested by Husum et al. ( 2), the first choice of treatment.
