✦ Synopsis
During the last decade conservative treatment of anal carcinoma based on radiotherapy has gained popularity. Radiotherapy is often combined with concurrent chemotherapy although no firm evidence exists that such combinations are more effective than radiotherapy alone. Treatment results with respect to anal sphincter function are generally better for small tumors. In an effort to improve the treatment results for locally advanced epidermoid anal carcinoma, a selection of patients with such tumors has been treated with induction chemotherapy comprising two or three courses of a combination of carboplatin 300-350mg/m z i.v. and 5-fluorouracil 1,000 mg/(m 2 x d) x 5 d prior to radiotherapy. Six female patients aged 37 to 74 have completed the regimen. All had complete tumor regressions at conclusion of radiotherapy. No recurrence has yet occurred but the followup period is still short (8-21 months). No interruption of chemotherapy due to toxic side effects was necessary and no severe toxicity registered. Further study is needed to evaluate the efficacy and toxicity of the regimen, but these early findings are promising.

Les r6sultats th&apeutiques en regard de la fonction sphinct6rienne anale sont g6n&alement meilleurs pour les petites tumeurs. Dans Un effort d'am61ioration des r6sultats th6rapeutiques pour les cancers 6pidermoides anaux localement avanc6s, une s61ection de malades avec de telles tumeurs a 6t6 trait6e par une chi-mioth6rapie d'induction comprenant 2 ou 3 cures d'une association de carboplatin 300-350 mg/m z I.V. et 5-fluorouracil 1,000 mg/(m 2 x d) x 5 d avant la radioth&apie. 6 femmes fig~es de 37 fi 74 ans ont termin6 ce traitement. Toutes eurent une disparition compl&e de la tumeur ~t la fin de la radioth6rapie. Aucune r6cidive n'est encore survenu mais la p&iode de suivie est encore courte (8-21 mois). Aucune interruption de la chimioth&apie par effet toxique n'a ~t6 ndcessaire et aucune toxicit6 s6v6re enre-gistr6e. Des 6tudes ult6rieures sont n6cessaires pour 6valuer l'efficacit6 et la toxicit6 de ce traitement, mais ces premiers r6sultats sont prometteurs.
