Clustering of sporadic cases of amyotrophic lateral sclerosis (ALS) in Caucasians is extremely rare. Cases were described in which environmental [2] and environmentaVoccupationa1

[4] causation was suspected. Paolino and co-workers 131 in a recent issue of the Annals described a very interesting case in which both husband and wife were affected by ALS. They also cited another, similar case [l]. Paolino and associates presumed an environmentalloccupational causation.

The authors failed to discuss certain points that seem important and relevant: (1) the prevalence and incidence of ALS in the community in which the couple lived in comparison to the rates in the whole region or country; (2) the consanguinity of the husband and wife; and (3) the presence of ALS in other family members, especially children, if any.
