✦ Synopsis

The D antigen of the Rh blood group system is of prime clinical importance because of its high immunogenicity. Alloanti-D acquired by transfusion or pregnancy may elicit hemolytic transfusion reactions and is a major cause of hemolytic disease of the newborn. Appropriate transfusion strategies and prenatal investigations depend on reliable RhD typing approaches. However, the latter are complicated by a vast number of RhD variants, such as weak D, partial D and DEL types. In addition, also silent RHD alleles and RhD chimerism and mosaicism have to be taken into account. Therefore, conventional serologic RhD typing is increasingly complemented by molecular RHD genotyping techniques.
