Between 1959 and 1965
82 patients with 84 intraarticular fractures of the distal end of the tibia were treated by open anatomical reconstruction and internal fixation. 50.3 months later the follow up displayed good functional results in over 73% of the cases, compared to about 40O/o after conservative treatment. 5 years later --on average 9 years after the accident --54 of these patients could be reviewed once more. Comparing the earlier results of these 54 cases with the present ones, an overall improvement of the function is found and only exceptionally an aggravation. It is believed, that especially in severely eomminuted fractures, open reduction with anatomical reconstruction and stable internal fixation is worthwhile, provided a good surgical technique with careful soft tissue handeling and a complete set of instrument is used.
