Complex response fluctuations represent a major complication of chronic levodopa therapy in patients with Parkinson's disease. Of those, the "wearing off' and "on-off'' phenomena are more widely recognized (1). However, two other subtypes of motor fluctuations, i.e., "delayed-on'' (increased latency from levodopa dose intake to start-up of clinical benefit) and "episodic unresponsiveness" (failure of a dose to turn patient on) are quite common and may significantly contribute to prolongation of daily "off' phases and to patients' functional disability (2,3).
