Evaluation of chemotherapy before limb-sparing surgery for osteosarcoma

✦ Synopsis

Preoperative chemotherapy before limbsparing surgery for osteosarcoma is used to reduce the size of the tumour, facilitate removal and prevent distant metastases. Fifteen patients with osteosarcoma were treated with six different protocols. Assessment of the clinical and radiological size of the lesion, of the histology of the resected specimen and of serum alkaline phosphatase levels were done. Greatest control of the tumour was obtained with the intravenous use of cisplatin, three times at intervals of three weeks before surgery, compared to the use of adriamycin or methotraxate in other protocols. Prompt surgery after preoperative chemotherapy and postoperative chemotherapy are advised.

R + s u m & La chimiothbrapie prb-opbratoire avant chirurgie conservatrice des membres pour ostbosarcome est utilisbe pour rbduire le volume de la tumeur, pour en faciliter l'exbrkse et pour prbvenir les mktastases. Quinze malades porteurs d'ostbosarcomes ont ~tb traitbs selon six protocoles diffkrents.

On a bvaluk le volume clinique et radiologique de la lbsion, l'histologie de la piOce rbsbqube et les taux sbriques de phosphatase alcaline. Le meilleur contr6le de la tumeur a ktb obtenu par l'emploi de la cisplastine intra-veineuse, en trois cures, dl trois semaines d'intervalle, avant l'opkration, comparativement fl l'utilisation d'adriamycine ou de mbthotrexate dans d'autres protocoles. On conseille d'intervenir rapidement aprks la chimiothbrapie prbopbratoire et de complbter par une chimiothbrapie post-opbratoire.
