RBD symptoms are well recognized in PD patients. Up to 33% of PD patients have either a clinical diagnosis of RBD or REM sleep without atonia by polysomnogram. 4 More importantly, a high proportion of RBD patients will develop PD in the future. A series of studies from different sleep centers have indicated that the risk of further neurodegeneration in patients with isolated RBD is ~5% per year. [5][6][7] Most often these cases are PD, but other degenerative disorders associated with alpha-synuclein pathology, including multiple systems atrophy (MSA) and dementia with Lewy bodies (DLB), also occur. RBD patients have evidence of dopamine deficiency on metabolic brain imaging. 8,9 Autopsies of RBD cases that subsequently developed dementia or parkinsonism almost universally show alpha synuclein pathology. 10 The authors and publisher regret this error.
