Recently, two reports of epithelioid hemangioendothelioma in pleural fluid appeared in Diagnostic Cytopathology. 1,2 Both include malignant mesothelioma in the discussion of differential diagnosis. One suggests that this vascular tumor may arise in, or immediately subjacent to, the pleura. 1 Our recent experience with a pleural effusion and subsequent vertebral aspirate from an 82-yr-old man illustrates some of the pitfalls associated with diagnosis of this uncommon tumor.

Briefly, fine-needle aspiration of a destructive lesion in the L2 vertebra was performed to confirm the clinical and radiological impression of metastatic neoplasm in an elderly man whose pleural biopsy had been diagnosed as biphasic
