✦ Synopsis

We read with great interest the study by Abdala et al., 1 which examines whether there is significant bacterial translocation during liver transplantation. This is of particular interest because we have recently shown that endotoxin can impair immune function in the context of liver disease, which may have significant ramifications for recovery. 2 We would like to congratulate the authors for undertaking such an innovative and difficult study. We would also like to address some points relating these interesting observations to the findings of our own work, possibly providing new insights into the pathophysiological basis of infection in liver dysfunction and recovery from ischemia/reperfusion injury.

Liver transplantation is associated with major shifts in central blood volume; therefore, monitoring preload is essential. As described in an interesting review by Rao, 1 much evidence derived from numerous experimental studies of alcoholic liver disease (ALD) support the association of endotoxemia with the initiation and progression of ALD. The review focuses on the role of endotoxemia and the mechanisms of gut barrier

Acute kidney injury (AKI) is common in liver failure prior to orthotopic liver transplantation (OLT) and may complicate the intraoperative course. We describe the logistics of intraoperative continuous renal replacement therapy (CRRT) during OLT and the associated clinical outcomes. We performed a r
