Endometriosis of the urinary bladder: Demonstration by sonography

✦ Synopsis


Endometriosis of the urinary bladder was first described in 1921.l Since then, approximately 130 cases have been reported in the literature. Sonographic demonstration of this lesion has been documented previo~sly.~3~ We report another case of endometriosis of the bladder diagnosed by sonography and review the literature.

CASE REPORT

A 28-year-old female presented with an intermittent history of hematuria and recurrent urinary tract infection for the last 7 months. She had two previous pregnancies. An intravenous urogram revealed a filling defect in the urinary bladder (Fig. 1). Sonography of the urinary bladder showed the mass to be solid and to arise from the posterior wall of the bladder extending transmurally, both inside the bladder lumen as well as outside (Fig. 2). Cystoscopy revealed a spherical, sessile mass with irregular surface located in the region of the bladder trigone. The overlying and adjacent mucosa was edematous. A transurethral resection of the mass was done, and frozen sections revealed it to be a n endometrioma (Fig. 3). The patient did well postoperatively and was discharged from the hospital on Danazol.

The patient returned t o the hospital 5 months later as she continued to have intermittent complaints of recurrent dysuria, hematuria, and urinary frequency. An intravenous urogram done at this time revealed that the mass had recurred and was now approximately half of its original size. Because of her persistent complaints, the patient underwent bilateral salpingo-oophorectomy and hysterectomy.
