Surgery is required if medical management fails, o r for severe endometriosisg-' '. Definitive surgery such as total hysterectomy and bilateral oophorectomy with excision and destruction of all known areas of endometriosis avoids the need for a subsequent laparotomy.
If classical symptoms are not elicited in the history the surgeon may be faced a t laparotomy with a lesion of the bowel closely mimicking carcinoma. I n such cases unnecessarily radical or inappropriate surgery may be performed. Previous gynaecological surgery should alert the surgeon to the possibility of endometriosis but in all cases a frozen section may provide positive confirmation of endometriosis.
