tients presenting later than 4 hr from the suspected onset of acute myocardial infarction were excluded from early thrombolytic trials because of the "wavefront hypothesis" examined in dogs, demonstrating that most necrosis was accomplished within the first few hours of vessel occlusion. When the paradigm was that the majority of benefit from thrombolytic therapy was from direct muscle salvage, rather than simply vessel patency with TIMI 3 flow, late patients were excluded from trials.
