✦ Synopsis


has developed a patient information pamphlet on advance directives. Under the leadership of its chairman, Steven C. Buchanan, M.D., the committee had adopted this project even before the passage of the Patient Self-Determination Act of 1990. It is thus able to offer the pamphlet as a model for any institution that might be searching for a way to comply with the patient information requirements of the new federal legislation. Institutions may feel free to use any of the brochure's content they find appropriate. (Some sections may have to be altered to conform to local laws.) If substantial portions are used, a suitable acknowledgment would be appreciated.

The pamphlet itself is a single four-fold sheet. Two panels contain the information copy printed below, and three panels contain a living will declaration, an optional proxy directive (allowed under the Arkansas advance directive statute) and an optional organ and tissue donation form.
