Benign cystic teratomas (dermoids) and cystadenomas are the two most common ovarian tumors. The combination of a cystic teratoma with a mucinous cystadenoma in the same ovary is not rare.' We present three cases of this entity and the corresponding sonograms.
CASE 1
A 27-year-old female presented with a 2-day history of increasing urinary frequency and vague lower abdominal discomfort. An excretory urogram revealed a large pelvic mass. She was admitted to the hospital, where the admission tests were noncontributory except for the pelvic examination. The uterus was deviated to the right by a large left-sided pelvic mass. Sonography confirmed the presence of a complex pelvic mass with a maximum dimension of 15 cm. The mass was predominately cystic with internal septations (Figs. 1 and2) and one circular echogenic focus (Fig. 3). Preoperative diagnoses of mucinous cystadenoma and dermoid cyst were entertained. At surgery, a uniform cystic mass appeared to be arising from the left ovary, and a left salpingoophorectomy was performed. Microscopic examination disclosed a smaller typical dermoid cyst partially lined by columnar epithelium and a large cyst lined by "picketfence" epithelium with clear mucin-containing cystoplasm. A diagnosis of mucinous cystadenoma and cystic teratoma of the left ovary was made.
