The case of a 21 -year-old man with dermatofibrosarcoma protuberans of the scalp that spread to deep cervical lymph nodes is presented. Available literature is reviewed, and the infrequency of metastasis is discussed. Dermatofibrosarcoma protuberans, first described in 1924 by Darier and Ferrand' as "progressing and recurring dermatofibroma," is a slowly but persistently growing tumor originating in the dermis.
