From 1982From through 1990, 350 , 350

patients (mean age 46 years) underwent delayed breast reconstruction after mastectomy by implantation of a silicone prosthesis under the muscle layer. Three hundred eighty-eight patients were reviewed at a mean of five years after surgery. The immediate complication rate was 12%. Late complications were noted in 29%, particularly discrepancies in volume and location of the implant and capsular contracture. A unit system was developed in order to analyze the achieved postoperative symmetry. The end result was graded on a 4-point scale. The objective findings were correlated with the patients' assessment. Excellent and good results were noted in 72% of the patients while 28% were classified as fair and poor.
