Case history: An 85-year-old white man with no history of heart disease was hospitalized because of symptoms suggestive of unstable angina pectoris. His first episode of angina occurred 4 days before admission, lasting for 2 hours and resolving spontaneously. He had a recurrence of the pain and went to the emergency department. An electrocardiogram during the pain was nondiagnostic and the symptoms required morphine and intravenous nitroglycerin for resolution.
