Summary. The course of an operation serves to demonstrate the danger of closed subcutaneous tenotomy of the lower insertion of the sternocleidomastoid muscle from the sternal and clavicular part thereof. Owing to the small distances, in the case of a baby, between the venter of the sternocleidomastoid muscle and the vessels behind it, the latter may be damaged, despite all precautions. Lesion of the subclavian artery will not be noticed outwardly; there is no appreciable visible bleeding; death occurs immediately due to exsanguination into the mediastinum, or within seconds due to heart failure or bleeding to death. It is recommended not to perform subcutaneous closed tenotomy of the sternocleidomastoid muscle at its lower insert any longer on account of the danger of venous and particularly arterial vascular lesion.
