C y s t o s a r c o m a phyllodes o f the prostate is a rare, relatively b e n i g n s a r c o m a o f the prostate. W e describe the m a g n e t i c r e s o n a n c e i m a g i n g findings in an unusual case o f c y s t o s a r c o m a p h y l l o d e s w h i c h resulted in e x t e n s i v e local r e c u r r e n c e and s a r c o m a t o u s degeneration. A l t h o u g h u n c o m m o n , radiologists should be aware o f the existence o f c y s t o s a r c o m a p h y l l o d e s o f the prostate.
