The causes of right iliac fossa pain are numerous, and it is the presence of localized peritonitis that determines the diagnosis of acute appendicitis. Various tests have been described's2 to elicit intra-abdominal inflammation, with equivocal results3. In this report, an evaluation of the 'cough sign' in determining a diagnosis of localized peritonitis is presented.
