Coronary Flow Parameters and Restenosis After Incomplete Covering of the Dissection With Stent

We have read with interest the article by Mun K. Hong et al. [ I ] and we would like to make some comments.

Coronary stenting is a well established treatment for the management of acute dissection in the advent of PTCA.

In this case report the authors claim to achieve an excellent angiographic result after stenting the dissected OM lesion with a 3,5 mm Gianturco-Roubin Flex-stent.

This was not followed by normalization of flow parameters, which became more abnormal than before stenting. The patient subsequently developed restenosis and the authors deem that the non-normalization of coronary blood flow parameters, despite the excellent angiographic result can predict restenosis.

In our opinion this angiogram did not appear to be excellent, as the authors state. We believe that the dissected segment was not covered in its full length.
