Shortly after reading the article by Burke and Jamieson (Br J Siug 1983; 70: 431-3), we treated a case of spontaneous aortocaval fistula which presented with a haematemesis. This seems to be a rare presenting feature and is not mentioned in Burke and Jamieson's article. We feel, therefore, it is worth recording.

A 68-year-old man was admitted to a medical ward with a 20 h history of nausea, back ache, central abdominal pain and a haematemesis. He has apparently vomited a few cupfuls of altered blood. He denied any recent significant drug intake or previous
