## Abstract Chronic total occlusions are typically difficult to recannalize especially when adverse angiographic morphologies are identified. We describe a case of chronic total occlusion crossing retrograde through the supplying collaterals followed by successful angioplasty and stenting. Β© 2006 W

β¦ Synopsis
This report describes a patient with a chronically occluded proximal LAD in whom antegrade flow was not reestablished by successful dilatation alone. Obliteration of retrograde collateral flow restored antegrade flow. This case demonstrates the potential detrimental effect of vigorous collateral flow on the patency of chronic occlusions after successful PTCA.
