โฆ Synopsis
A 61-year-old woman presented with weight loss, melena, and iron deficiency anemia. Roentgenologic examination aroused suspicion of tumor in the duodenum. Laboratory findings indicated the existence of hyperparathyroidism. At laparotomy, an infrapapillary duodenal carcinoma was found and resected. Microscopic examination showed a moderately well-differentiated adenocarcinoma without involvement of papilla or pancreas. Postoperatively, the patient developed pancreatitis and subsequently peritonitis which led to death. Autopsy showed a parathyroid adenoma that microscopically appeared to be composed of chief cells in acinar arrangement. This adenoma obviously formed the basis for the hyperparathyroidism.
