was to reduce PWP; nitropmsside reduced PWP to a greater extent, and also reduced mean arterial pressure and increased cardiac index. Pulmonary vascular resistance was only reduced by nitroprusside. In cases of equally reduced PWP, cardiac index rose to a greater extent with nitropmsside. This suggests that nitropmsside has a more beneficial effect on hemodynamics and ventricular function in patients with acutely decompensated chronic congestive failure. [Editor's note: The logistics of setting up a PWP monitor and a nitroprusside drip with subsequent monitoring of such patients is impractical in the ED setting, even though reduction in afterload may be more beneficial than reduction in preload. Such patients should be expeditiously transferred to an ICU where such therapy can be instituted.
