Coarctations of t h e abdominal a o r t a involving its inter-and suprarenal segment a r e usually operated on t h r o u g h a thoracoabdominal approach. W e describe a modified access of t h e lower p a r t of t h e descending a o r t a t h r o u g h t h e diaphragm. I t was successfully used for the treatment of two aortic coarctations 5 and 6 years ago.
