✦ Synopsis


Kazuhiro Sat0 (Department of Gastro-BACKGROUND. Early diagnosis and complete removal of pancreatic adenocarcinoma [PC) is es5ential to improve its poor prognosis. It is necessary tu finil PC when it is sinall 2nd at an early stage if surgical treatmenr is to be successful. This is an investigation of the clinicopathologic features of small N:, METHODS. 'l'hirty-one rrsected and hislopntlrologicail!, exaniiircd cases of small PC, 2 cm or l e s in greatest dimension, were collected from i institutions between 1989 and 1994, and their clinicopathologic features were analyzed in detail.

RESULTS.

Eightecn patients (58.1r0) had some symptoins or signs at the tiiiie of diagnosis, however. the other 13 (41.9%) had no syniptonis. 'There was no sensitive blood test for the detvrtion of small PC. l h c drtrction rate? of the pmcreatic: 1iinror miss o n [iltrasoriograptry, endoscopic iiltrasoririgraplry, arid conipiitcd 10mographywere 6$.5?6 (20/31), 73.7% [14/19), arid 64.5% (20/31), respectively. Only 13 patients (41.9%) were in International IJnion Against Cancer (UICC) 5tage I, and the other 18 were in Stage 111 or IV due to lymph node inetastasis or peritoneal dissemination. The overall 4-year postoperative survival rate was 54.5'10 and that of patients in Stagc I w;rs 77.9?&.

CONCLUSIONS.

.rr~ detect a small I)(:, a ~Iiorougli exarniiiation iiiaq' be rcquired in patients with slightly abnormal findings on laboratory tests or imaging modalities, even if the patient is asymptomatic. PC 2 cm or less and belonging to Stage I may be regarded as a n early carcinoma, because of its good prognosis. Cancer 1996;
