Subdural effusions of 24 children were cytologically tested with the sediment chamber process according to SAYs. The curve obtained from the puncture treatment gives an insight into cellular response in subdural effusions. New ideas are given, and old findings are substantiated by the large number of results. Postmeningitic effusions often permit the recognition of a granulocytic cell pattern. If the asepsis is strictly observed during the puncture treatment, infections of the effusions seldom occur; they can, however, be recognised from the cell pattern in time and properly influenced antibiotically. A rise in the number of neutrophil granulocytes to values over 100/#l must be taken as a sign for the application of antibiotic therapy. Eosinophil and plasma-cellular reactions are signs that point to local immunisation processes. If the cell patterns are identical in the effusions of both sides, it could be a sign that the reaction processes are being controlled by unknown mechanisms. With all the patients the round-nucleated histiocytes are distinguished from the monocytic elements by strong phagocytosic and storage activity. The large vacuoles contained in many histiocytes are mainly residues of phagocytosis of erythrocytes.
