S u m m a r y . E l d e r l y patients with e n d -s t a g e renal disease often r e m a i n on dialytic t h e r a p y b e c a u s e they are at increased risk for m o r t a l i t y and morbidity. We p l a c e d 24 c a d a v e r k i d n e y transplants into 24 patients aged 6 5 -7 4 years b e t w e e n S e p t e m b e r 1, 1985, and A u g u s t 31, 1995. Rates o f patient and graft survival were c o m p a r e d with those of 404 c o n c u r r e n t first c a d a v e r -k i d n e y transplant recipients b e t w e e n the ages o f 20 and 44 years. The 5 -y e a r rates o f patient and graft survival were not significantly different (86% versus 92% and 77% versus 63%, respectively; study group p r e s e n t e d first). Prim a r y c a d a v e r k i d n e y transplantation can be successfully p e r f o r m e d in patients older than 65 years w h e n a selection algorithm is a p p l i e d to e x c l u d e active infection, active m a l i g n a n c y , unsuitable a n a t o m y for technical success, high p r o b a b i l i t y of operative mortality, and n o n c o m p l i a n c e . Pelvic arteriosclerosis and l o w e r urinary tract a b n o r m a l i t i e s can cause intraoperative technical p r o b l e m s .
