In cases where the stricture is so long (for example > 5 cm) that mobilization from the perineum alone is unlikely to be successful, then an abdominoperineal approach would be recommended as the primary procedure.
The technique described here is useful and should be included in the armamentarium of the pouch surgeon. However, it remains more efficacious to avoid complicated anastomotic stricture by meticulous preparation of the bowel ends at the time of anastomosis, ensuring a tension-free ileal pouch anal-anastomosis and prevention of pelvic sepsis.
