To provide seamless care, the various sectors namely primary care, hospital services community care, and the private sector, need to work together. The book ยฎrst makes the case for the elderly as a speciยฎc group and then goes on to discuss how the dierent sectors ยฎt together to provide seamless care. This is followed by a description of the individual roles of diering professionals who are involved with the care of the elderly, but more importantly gives practical guidelines to creating successful multidisciplinary teams which will continue to develop and hopefully improve with continuing education.
The delivery of high quality care across the various sectors by the multidisciplinary team occurs within organisations that are constantly changing. This, the subject of the last section of the book addresses such issues as to how to develop organisations for quality and planning for the future care of the elderly. This is a well-written, well-organised and also focussed book, founded on a wide knowledge base. I
