Motivated by the problem of loosening and breaking of the alloplastic hip joint prostheses the forces and biomechanical relations of the different effects were investigated. It is shown that not the choice of the material is the main question but the stabilized and durable anchorage of the prosthesis in the bone achieved by a bone-adequate kind of transmission of load. These forces which affeet the anehorage of the prosthesis in the bone result in one part from the natural construction of the body but to one part from avoidable characteristics of the eonstruction of the prostheses. The loosening and breaking of the prostheses are caused by the unphysiological forces in medial direction. These forces were investigated in detail. Special proposals to avoid or reduce these unfavourable forces are given.
Zusammen/assung. Veranlaßt durch die beim alloplastischen I-Iüftgelenkersatz auftretenden
Probleme der Prothesenlockerungen und -brüche wurden die an der Prothesenverankerung auftretenden Kräfte und ihre biomeehanisehen Auswirkungen untersucht. Es wird gezeigt, daß nicht primär die M~terialfrage im Vordergrund steht, sondern daß die durch eine knochengemäße Xraftübertragung erzielte stabile und dauerhafte Verankerung der Prothese im Knochen von grundsätzlicher Bedeutung ist. Die auf die Prothesenverankerung einwirkenden Kräfte resultieren teils aus dem natürlichen Körperbau, teils aber auch aus vermeidbaren Konstruktionsmerkmalen heute üblicher Prothesen. Als Ursachen der Prothesenlockerungen und -brüche ergibt sich vor allem die nach medial gerichtete unphysiologisehe t~elastung des Prothesenlagers, deren Einzelkomponenten untersucht werden. Zur Ausschaltung bzw. Reduzierung dieser Kräfte werden spezielle Vorschläge mitgeteilt.
