Abstract

Unclear or confusing process control system displays in the control room can cause process safety incidents or near misses. An incident is described in which two very similar displays on a process control system resulted in the inadvertent operation of several valves on the wrong equipment train, causing a batch to rapidly boil over into the reactor overhead system.
