Perhaps an ideal study using myelography would not be permitted by a human subject committee because of patient risk. However, a future study should at least call for the performance of plain radiographs and bone scintigraphy in all cases. An alternative is to use nuclear magnetic resonance imaging as the gold standard [ 4 ] . Finally, it would be helpful for the generalist to glean from such a study which types of 'cancer are more likely to result in false negative index tests for epidural metastases. In other words, when should one still obtain a myelogram despite a normal physical examination, negative plain films, andor a negative scan?
