✦ Synopsis


A total of 42 patients with biopsy-proven Hirschsprung's simple and complication-free procedure. The results disease were treated by anorectal myotomy or myectomy, compare favourably with those of other operative alone or combined with anterior resection. Myotomy Is a procedures for this condition.

In 1964, Bentley' described a procedure that he called posterior anorectal excisional myotomy to treat 22 children with chronic constipation. This operation involved excision of a 5-7-mm full-thickness strip of rectal wall in the posterior midline that included the internal anal sphincter. The resultant defect was repaired with catgut. The strip excised could be as long as 10 cm. Ten of the patients had Hirschsprung's disease; five did well with myotomy alone but four subsequently underwent a Swenson procedure. Bentley's first myotomy was performed in 1956.

In 1965, Duhamel' at a seminar on pseudo-Hirschsprung's disease described a procedure that he called submucous sphincterotomy. The anal canal was dilated and, through a transverse incision at the mucocutaneous junction, the mucosa was raised and a strip of internal sphincter and circular muscle of rectum approximately 1 cm wide and 5 cm long excised. The transverse incision was closed with catgut. Duhamel likened the procedure to Ramstedt's pyloromyotomy and Heller's operation for achalasia. He claimed the operation to be 'quick, simple and free of complications'. Ten of these patients had Hirschsprung's disease. Two failed to respond to myotomy and
